I understand and acknowledge that the activities of playing pickleball carry inherent risks, including but not limited to physical or psychological injury, pain, suffering, illness, and death. I am voluntarily participating in these activities entirely at my own risk and knowingly assume all such risks, both known and unknown. I hereby release, waive, discharge, and covenant not to sue CBCV Ventures LLC dba Long’s Sideout Sports and Spirits, owners, directors, officers, employees, and agents from any and all claims, injuries, and damages arising from my participation. I agree to indemnify and hold harmless CBCV Ventures LLC dba Long’s Sideout Sports and Spirits from any and all claims, actions, suits, costs, expenses, damages, and liabilities, including attorney's fees, brought as a result of my involvement in the event(s). I grant CBCV Ventures LLC dba Long’s Sideout Sports and Spirits permission to use my name, image, and likeness in any lawful manner connected with the event, including promotional and marketing materials. I certify that I am physically fit to participate in this activity and have not been advised not to participate by a qualified medical professional. This waiver shall be governed by the laws of the State of Ohio.
